Illinois DOR Releases Income Tax Installment Payment Plan ACH Debit Payment Authorization Form CPP-1-A

The Illinois Department of Revenue (DOR) June 1 released Form CPP-1-A, on ACH debit payment authorization for installment payment plans, for individual income and corporate income tax purposes. The DOR requires information on: 1) the identity of the taxpayer and spouse; 2) the identity of the business or organization; 3) ACH payment frequency; and 4) financial institution and account information.
