The Kentucky Department of Revenue Sept. 1 released Form K-4, Withholding Certificate, with instructions for corporate income and individual income tax purposes. All Kentucky wage earners are taxed at a flat 4.5 percent tax rate with an allowance for the standard deduction. The instructions for employees provide information on exemptions from withholding and requirements for such exemptions. The instructions for employers provide that the form is required only to document that an employee has requested an exemption or additional withholding in excess of the calculated amounts.
