The Missouri Department of Revenue Dec. 24 released Form 5695, Qualified Health Insurance Premiums Worksheet, for individual income tax purposes. Taxpayers need to complete the worksheet and attach to Form MO-1040, along with proof of premiums paid, if health insurance premiums paid were included as an itemized deduction or if health insurance premiums were withheld from social security benefits.
