Vermont Department of Taxes Publishes Fact Sheet on Health Care Fund Contribution Excise Tax Assessment

The Vermont Department of Taxes Feb. 1 published a revised fact sheet on the health care fund contribution assessment. The excise tax fact sheet provides information to help taxpayers determine which employees are considered covered and which are considered uncovered by health care. The fact sheet includes instructions for reporting and paying the assessment, including basic definitions and details for withholding reconciliations. The forms taxpayers and employees need to report health care contributions are: 1) Form HC-1, Health Care Fund Contribution Assessment; 2) Form HC-2, Declaration of Health Care Coverage; and 3) WHT-436, Quarterly Withholding Reconciliation.
