Washington DOR Releases Real Estate Excise Tax Affidavit Form 84 0001a (Form 2 of 2)

The Washington Department of Revenue Sept. 8 released the Real Estate Excise Tax Affidavit, Form 84 0001a, only for sales in multiple location codes on or after Jan. 1, 2023. The form includes fields to provide: 1) the seller or grantor’s name, mailing address, and contact number; 2) the buyer or grantee’s name, mailing address, and contact number; 3) the name and address for sending property tax correspondence; 4) a list of all real and personal property tax parcel account numbers and assessed values; and 5) land use codes.
