Final versions of 2019 health-care forms that are to be filed under the Affordable Care Act were released Dec. 9 by the Internal Revenue Service.
The forms are 1094-C, Transmittal of Employer-Provided Health Insurance Offer and Coverage Information Returns; 1095-C, Employer-Provided Health Insurance Offer and Coverage; 1095-B, Health Coverage; and 1094-B, Transmittal of Health Coverage Information Returns.
Instructions for Forms 1094-B and 1095-B also were released. Instructions for Form 1094-C and 1095-C were released Dec. 6.
